Office Depot and some craft store carry kits to make personalized mouse pads at home. Some are iron on type things and others have a thick piece of plastic (clear) that’s sticky. You just put the design on the pad and cover it with the plastic.
Office Depot’s copy & print center can send them out to be done also, but I don’t know how long that takes.

cafepress.com can do customs (and you can create a sellers account and sell too), Vistaprint.com I think does them. OR you can alsways buy a blank white mouse pad, transfer paper and make one yourself at home! Make sure you set your printer settings to transfer if your using anything with text on it.
