Question : How do I file a motion with the “proper wording” for a pro-per lawsuit?
I am the Plantiff and this is for Negligence, slip and fall, Premises Liability, damages, pain & suffering.

The proper wording depends on your state. But generally, to establish negligence, the plaintiff needs to establish that the defendant owed them “a duty of care” which they “breached” and that the breach was the “proximate cause” of the “harm” the plaintiff felt.

So you slipped on the floor of a business. They owed you “a duty of care” to keep the floor clean or free of debris/water/other dangerous items or spills, in order to protect their customers from injury. They “breached the duty of care” because they allowed the floor to be wet/debris-riddled/whatever.” You were injured and the “proximate cause” of those injuries was the business’s failure to keep their floor clear, and that caused you “harm” in the form of xxx.
